Who Can Apply For Adult Day Care? 

  • Our day care facility is open to individuals who wish to use both private funding or Medicaid. In both instances, eligibility requires that the individual is at least 18 years old with a medical condition requiring treatment or rehabilitative services prescribed by a physician; or has an existing physical or mental impairment which handicaps activities of daily living; or has a reasonable expectation that preventative services will improve or maintain their present level of functioning.

Where Can I Find Out About Funding Options To Pay For Adult Health Day Care? 

  • Persons who are financially eligible for Medicaid and require assistance owing to medical, cognitive, and physical challenges should contact the Ridgeland Community Long Term Care (CLTC) office @ 843-521-9191; Physical Address: 10175 S Jacob Smart Blvd, Ridgeland, SC 29936; Website: https://www.scdhhs.gov/; or consider the South Carolina Department of Disabilities and Special Needs (DDSN) to facilitate the process of obtaining an Intellectual Disabilities/ Related Disabilities (IR/RD) waiver. How Do I Start The Application Process For Adult Health Day Care? 

  • If you intend to use your own private health insurance or funding, have your physician complete our physician assessment form and return it to our main office location. Upon receipt, you will receive a response from our office within two (2) business days. If you intend to use your Medicaid waiver, simply follow the same above steps. Once you have provided us with a completed physician’s assessment form, we will contact your designated caseworker. Upon authorization (which generally occurs within 3-5 business days), you will be qualified to receive adult day care services.

What Are Your Hours Of Operation? 

  • In order to accommodate the schedule of working families, Tucker’s Adult Health Day Care is open from 7:30am to 5:30pm Monday through Friday.

Is Transportation Available?

Door To Door Transportation 

We readily provide transportation to and from out facility for clients who reside within a 15 miles radius of our 112 Carn Street location. If you live beyond our designated perimeter, a Medicaid waiver is available for a full-range of transportation services. 

What Is The Staff To Client Ratio At Your Facility?

  • The South Carolina Department of Health and Environmental Control requires a minimum staff to client ratio of 1:8. Our ratio is typically 1:6.
